What am I required to bring with me to the PA state exam?

You must bring two forms of identification with you to the PSI testing center.
One must be a valid form of government issued identification (driver's license, state ID, passport, or military ID), which includes your signature and your photograph or complete physical description.
The second identification must include your signature and pre-printed legal name.
The name on your identifications must match the name you entered on the Examination Registration Form and your Registration Confirmation Notice.
If you do not have the required identification, you must call PSI at least 3 weeks prior to your exam date to arrange a way to meet this security requirement.
