What am I required to bring with me to the VA state exam?
You must bring two forms of valid (not expired) identification to the PSI testing center. One ID must be a valid government-issued ID, such as a driver's license, state ID, passport, or military ID, which includes your signature and photograph. The second ID must include your signature and preprinted legal name. The legal name on your two IDs must match the name you used on the Examination Registration Form, otherwise you will not be permitted to take the exam and your exam fee will be forfeited.
